Our nurses work alongside social workers, chaplains, aides, providers, and leadership to create individualized care plans that support the physical, emotional, spiritual, and psychosocial needs of every patient. What You'll Be Doing Provide skilled hospice nursing visits in homes, assisted living communities, memory care settings, and other community-based environments. Perform comprehensive patient assessments and ongoing clinical evaluations. Manage pain, symptoms, wound care, and comfort-focused interventions. Educate patients and families regarding disease progression, symptom management, medications, and hospice services. Develop and update individualized plans of care. Collaborate closely with social workers, chaplains, hospice aides, providers, and other interdisciplinary team members. Participate in care conferences and interdisciplinary team discussions. Support patients and families through end-of-life transitions with compassion and professionalism. Maintain accurate and timely clinical documentation. Participate in an on-call rotation supporting continuity of care. What We're Looking For Active Minnesota Registered Nurse (RN) License Experience in hospice, palliative care, oncology, home health, ICU, med surg, long-term care, or other relevant clinical settings preferred. Strong communication and patient education abilities. Excellent clinical judgment and assessment skills. Ability to work independently while collaborating effectively within a team environment. Strong emotional intelligence and compassion. Reliable transportation and willingness to travel throughout Southern and Southwest Minnesota. Join A Team That Makes A Difference Together.
