What You'll Do:
Perform routine assessments and monitor health changes Administer medications and treatments per care plan Support patients with education, safety, and daily living needs Document visits accurately and communicate updates to the care team
What You Need:
Active LPN license in Minnesota At least 1 year of experience in home care or a related setting Strong communication and critical-thinking skills Ability to work independently with confidence Reliable transportation for travel between visits
