Admissions Coordinator Foxwood Springs - 2.6 Raymore, MO Job Details Full-time 15 hours ago Qualifications Skilled nursing facility experience Medical law Long-term care facility experience LPN Full Job Description We are currently seeking an experienced LPN to join our team as an Admissions Coordinator at Foxwood Springs. As an Admissions Coordinator, you will be responsible for managing the admission process of new residents, ensuring that they receive the highest level of care possible.
Responsibilities include:
Managing the admission process, including coordinating with families, physicians, and other healthcare providers Conducting initial assessments of potential residents to determine eligibility for admission Coordinating with other departments to ensure that the resident's needs are met Providing support and guidance to families throughout the admission process Maintaining accurate records and documentation
Requirements:
Active LPN license with minimum five years of SNF experience and critical skill set experience required.
