Description of the role:
The Intake Coordinator will be at the heart of welcoming new clients to Advantage Home Care, ensuring that every referral and inquiry is handled with compassion, efficiency, and professionalism. They will play a key role in driving client onboarding, streamlining communication between referral sources, families, and care teams, and ensuring that all intake processes are thorough, compliant, and data informed. Their commitment to service excellence and attention to detail will support the agency's growth, while their collaboration across departments will strengthen internal operations and client satisfaction. Together with the broader team, the Intake Coordinator will help build meaningful connections, deliver a seamless onboarding experience, and ensure every new client journey begins with clarity, care, and confidence. Main responsibilities you will be assigned to: Respond promptly and compassionately to all new client inquiries and referrals. Gather and verify client information, ensuring accuracy and completeness. Walk potential clients through assessments over the phone to help prequalify. Communicate effectively with referral sources, families, and care teams. Ensure all intake documentation meets compliance and regulatory standards. Stay updated on funder guidelines and state requirements impacting eligibility. Input and track client data in the agency's systems with accuracy. Collaborate with scheduling and staffing teams to align caregivers with client needs. Monitor the progress of each intake to ensure smooth, timely onboarding. Provide ongoing support and follow-up to clients during the onboarding phase. Other relevant duties assigned by management, as needed. To be successful in this role: Stay detail-focused : Accuracy in data and documentation prevents future issues. Communicate clearly : Keep referral sources, clients, and staff aligned.

Balance care with efficiency:
Balance efficiency with compassion in every client interaction. Preferred qualifications that will help you in this role: High school diploma or equivalent required. Strong communication skills (phone, email, in-person). Experience navigating funding sources and service eligibility. Basic computer skills, including data entry, email, and navigating client management systems. Previous experience in customer service, home care, or office administration a plus but not required. 8:00 am - 4:30 pm, Monday-Friday Salary /
Pay Rate:
$35,000-$40,000
