We want to hear from you. We're looking for a Nurse Practitioner or Physician Assistant/Associate to join our dynamic, multidisciplinary team at ouroutpatient diabetes clinic— where evidence-based medicine meets genuine, patient-centered care. This is more than a clinical role; it's an opportunity to become an integral part of a community that's transforming how patients live with chronic disease. As a key member of our team, you'll:
  • Lead meaningful patient visits and craft individualized care plans targeting glycemic control, disease prevention, and long-term wellness
  • Partner with endocrinologists, diabetes educators, dietitians, and nursing staff to deliver truly holistic care
  • Empower patients and their families with the knowledge and confidence to manage diabetes through education on lifestyle, medications, and self-care
  • Order and interpret labs, fine-tune treatment regimens, and track outcomes that matter
