We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our multidisciplinary team in our outpatient diabetes clinic. This is an excellent opportunity to work with a collaborative, patient-centered group dedicated to delivering comprehensive, evidence-based care to individuals living with diabetes and related endocrine conditions. This position is based in Keene, NH, a beautiful college town in southwestern New Hampshire with outdoor recreational opportunities, cultural venues, and a vibrant downtown.
Key Responsibilities:
  • Conduct patient visits and develop and implement individualized care plans focused on disease prevention, glycemic control, and long-term health
  • Collaborate closely with endocrinologists, diabetes educators, dietitians, and nursing staff to deliver holistic patient care
  • Educate patients and families on diabetes management, lifestyle modifications, and medication use
  • Order and interpret labs, adjust medications, and monitor treatment outcomes
  • Participate in quality improvement initiatives and contribute to program growth
Qualifications:
  • Nurse Practitioner or Physician Assistant with ability to obtain/maintain New Hampshire licensure
  • National certification as a PA, FNP, or AGNP
  • Prior experience in endocrinology, diabetes care, primary care, or related specialty
  • Strong clinical judgment, communication, and patient education skills
