JOB SUMMARY
The medical assistant will provide clients with health care navigation in the community within the CCBHC/BHH/Virtual exam room model. The medical assistant will provide structured advocacy, social support, and navigation to high-risk patients in order to help them reach their health goals. The medical assistant will work closely with medical providers, primary care teams, and social services agencies to provide short term care coordination and connection to resources for clients in efforts to improve their health and general well-being through education and provision of coordination of care and services. Requirements Responsible for creating connections between vulnerable populations and the health care system, facilitating the coordination and transition of care for clients, Assists with enrollment in programs and benefits for which clients are eligible Encourages cultural competence among health care professionals serving vulnerable populations, Builds capacity within the clinic and the community to address health issues and serves as an advocate for vulnerable populations within the health care system and the community at large. Responsible for delivering case management/counseling services for clients and families with an emphasis on navigating the crisis system and linking clients to long-term services. Greet and room patients at various sites, including virtual exam rooms, and in home/community Obtain and document patient medical history, vital signs, current medications, medication dosage and frequency, patient allergens in EHR Responsible for scheduling and resolving of Dr. Appointments and follow up appointments in coordination with administrative staff Assist with retrieval of collaborative client care documents from community partners/client consents/medication refills Educates and advises patients on specified medical issues within established parameters Data entry of CCBHC client measures into clinical record Assist with medication prior authorizations Maintain accurate and strict confidentiality of patient information in electronic and/or manual systems as established by the appropriate protocols, regulations, and policies. Contacts and/or meets with eligible patients to enroll them in the program Performs, when possible, appropriate Social Determinants of Health Screening Documents all interactions with patients and potential patients in electronic system(s), as directed Analyzes health system data to identify patients who are at eligible for the program Acts as a Liaison between staff, to facilitate care management activities Accesses and runs reports to facilitate data collection for program operations and evaluation Prepares reports and documents as needed or requested Attends regular team meetings, both internal and external Maintains privacy of Protected Health Information as required by law and CC policy Performs other duties as reasonably assigned by Director of Crisis Services
