Intake Coordinator

New Horizon Counseling Center is seeking experienced Intake Coordinators for its out-patient clinics. The Intake Coordinator will serve as the initial contact with referring hospitals and their in-patient social workers who will refer patients to our Out-Patient Mental Health Facility, which includes clients with mental health and substance abuse issues. The responsibilities of the Intake Coordinator will include the ability to:

• Function as initial contact for all referring agencies in the community.
• Assist clients with non-active Medicaid through initial entitlement process
• Complete client detail intake (pre-screenings, Medicaid eligibility, Health Homes consents and intake assessment
• Maintain good working relationships with all referring staff from hospitals and agencies throughout the community.
• Perform telephone and walk-in screenings for clients seeking services from the Community.
• Work closely with our Clinical Staff, Billing Department, Business Development, Care Coordination Teams and with the Medical Practice located in our building to share and refer clients.
• Set up Patient accounts and schedule clients through our Electronic Medical Record using the Accumed Program.
• Retrieve faxes and emails to upload client information into our EMR and filing program, Ebridge.
• Retrieve clinical documentation on all existing clients that are hospitalized and obtain discharge paperwork when clients are released.
• Maintain filing system and electronic documentation
• Exhibit a friendly, enthusiastic and engaging demeanor with the ability to understand and explain all our programs in detail
• Perform other duties as requested
