Position Overview:
We are seeking an experienced Patient Service Coordinator to join our team. The ideal candidate will have a background in service coordination, staffing, or client support within a home care agency or nursing home setting. This is a full-time position, and we are eager to fill it immediately with a compassionate, upbeat, and team-oriented individual.
Key Responsibilities:
Manage a reasonable-sized caseload, ensuring services run smoothly and meet client needs. Communicate effectively and professionally with clients, family members, and caregivers to provide outstanding customer service. Coordinate with a responsive team of nurses and caregivers available for guidance, support, and assignments. Use HHA Exchange software to manage client schedules, documentation, and communication. Participate in weekly staff meetings, offering insights and updates on caseloads and service requirements. Answer phone calls in a courteous, professional manner and maintain organized, thorough documentation.
Qualifications:
Experience as a Service Coordinator, Staffing Coordinator, Patient Service Coordinator, or HHA Supervisor in a home care agency or nursing home. Familiarity with HHA Exchange software and general computer proficiency. Excellent communication, documentation, and follow-up skills. Ability to solve problems efficiently and maintain an upbeat, professional attitude. Strong teamwork skills, with the ability to collaborate effectively with an amazing team of caregivers and nurses. A commitment to providing top-notch customer service and a positive experience for clients, families, and team members.
