• JOB SUMMARY
The Director of Patient Services provides strategic and operational leadership across a multi-licensed Home Care Agency (LHCSA). The Director of Patient Services oversees Clinical and PCA/HHA service delivery to ensure consistent, compliant, and high-quality patient care across all licensed entities. This role partners with executive leadership to standardize operations, optimize performance, and align care delivery with NYS DOH, MLTC, Managed Care, and all accreditation requirements. ESSENTIAL DUTIES AND RESPONSIBILITIES
Standardize policies, workflows, and care delivery models across all LHCSA licenses and regions Ensure consistency in patient care, documentation, and compliance practices across entities Oversee intake, admissions, authorizations, and care coordination in alignment with payor timelines Ensure clinical quality across skilled nursing services, including assessments, supervisory visits, and care management Monitor case coverage, service continuity, and escalation resolution Ensure full compliance with NYS DOH, OMIG, MLTC, and accreditation standards Maintain audit readiness, documentation integrity, CHRC compliance, and credentialing requirements Lead investigations and resolution of incidents, complaints, and compliance risks Lead QAPI initiatives and continuous quality improvement efforts Analyze trends in patient outcomes, complaints, and attrition to drive corrective actions Deliver executive-level reporting and performance insights Supervise and develop Supervising RNs, Administrative Nursing staff, and interdisciplinary clinicians Partner with HR and Recruitment on hiring strategy, onboarding, and retention Drive accountability, training, and performance management across teams Serve as liaison to MLTC plans, Managed Care Organizations, and referral sources Manage escalations related to patient services, authorizations, and service delivery Support expansion across new licenses, regions, and service lines Lead program development, emergency preparedness, and operational improvements Perform other duties assigned as needed Qualifications/Education:
NYS Licensed Registered Nurse (RN) required; must possess 5 yrs. of work experience. BSN required; MSN preferred in public administration, health care or community services; 5+ years healthcare experience, including 2+ years in leadership Experience in LHCSA, home care, MLTC, or multi-site operations strongly preferred Demonstrated proficiency in program management and supervision.
