Job Description

Intake Coordinator Full-Time, Nights 7pm-7am, Panama Schedule. Nationally growing hospital group has an immediate opportunity for an Intake Coordinator in the Dayton area. The Intake Coordinator responds to inquiry calls, conducts initial intake assessments, provide referrals, and coordinates the admissions process.

Medical, Dental, Vision Coverage 401 (k) Retirement Savings Plan w Tuition Reimbursement Daily Pay Paid Time Off Intake Coordinator Responsibilities Responds to inquiry calls, completes inquiry forms accurately, schedules assessments in a timely manner and documents follow up calls made regarding pending assessments. Conducts assessments at facility or other locations, completes Comprehensive Interdisciplinary Assessments Initial Triage thoroughly. Refers patients who are not appropriate for facility programs to appropriate resources. Coordinates pre-certification process, admissions process for patients admitted to facility programs, including admission forms, CIAs, and documents pertinent UR information. Delivers patient services in a respectful manner which promotes dignity and patient satisfaction. Identifies safety needs including ability to detect possible signs of abuse and neglect and report same to clinical staff. Assess and interprets age and disability needs to assess level of care needed and made appropriate referrals based on same. Intake Coordinator Requirements Bachelor's Degree in Psychology, Social Work, Counseling or related field or an LVN/RN or equivalent number of years of experience preferred. One year experience in Mental Health/Chemical Dependency field Must have basic knowledge of Chronic Mental Illness and clinical therapy modalities Must be knowledgeable concerning
