Job Description

Join Our Healthcare Team! We are seeking an experienced, motivated, and customer-focused Admissions Coordinator to manage the admissions process and help ensure a seamless transition for residents entering our skilled nursing and rehabilitation communities. This role is responsible for overseeing admissions for two nursing home facilities and is ideal for a professional who enjoys building relationships, coordinating care transitions, and providing exceptional customer service to residents, families, and referral partners. Position Summary The Admissions Coordinator is responsible for overseeing all aspects of the admissions process for two nursing home facilities, from initial inquiry through admission. This position works closely with hospitals, healthcare providers, residents, and families to facilitate admissions while supporting census growth and maintaining compliance with regulatory requirements. Key Responsibilities Manage the admissions process for two nursing home facilities from referral through resident admission. Coordinate tours for prospective residents and families. Review referrals and collaborate with the interdisciplinary team to determine admission appropriateness. Build and maintain relationships with hospitals, physicians, case managers, discharge planners, and community partners. Ensure timely completion of admission paperwork and required documentation. Maintain accurate admissions records and tracking reports. Assist with census development and occupancy growth initiatives for both facilities. Serve as a liaison between referral sources, residents, families, and facility staff. Communicate admission decisions and updates in a timely and professional manner. Coordinate admissions activities and priorities across both locations. Ensure compliance with all applicable federal, state, and facility regulations. Maintain confidentiality and uphold HIPAA standards. Qualifications Previous experience in admissions, healthcare marketing, business development, customer service, or long-term care preferred. Knowledge of Medicare, Medicaid, managed care, and insurance processes preferred. Excellent communication, interpersonal, and presentation skills. Strong organizational and time-management abilities. Ability to effectively manage multiple priorities across two facilities. Proficiency with electronic medical records (EMR) systems and Microsoft Office applications. Ability to work independently and as part of a team. Bachelor's degree in Healthcare Administration, Business, Marketing, or a related field preferred.
