Job Description

Are you looking for a fast paced role in healthcare? Come join our team in Admissions! The Admissions Coordinator is responsible to assist with coordination of all skilled nursing and personal care admissions in a strong team environment. This position is Monday through Friday 830am-5pm with a weekend require approximately once every 6 weeks. Qualifications of the
Admissions Coordinator:
Minimum one year experience in related health care environment in long term care or in acute care working with the geriatric population; nursing or social services background preferred. Excellent skills in public relations, communications, time management and organization required. Self-directed and motivated to perform job duties with minimal supervision. Commitment to serving the senior population in a progressive health care environment. Computer experience. Essential Duties and Responsibilities of the
Admissions Coordinator:
Compile required preadmission criteria including application, facility tour, medical and financial information, nursing report, and insurance verification. Communicate and maintain open lines of communication with residents, families, referral sources, physicians, and staff. Complete admission paperwork timely and accurately including, but not limited to, resident record of admission, admission agreement and additional required admission forms. Obtain required documentation, ie. Insurance cards, Living wills and Power of Attorney forms. Meet with resident and family to answer questions, provide support and to transition to nursing staff. Market Concordia services including participation in community health fairs and organizational events. Demonstrate knowledge of insurance plans, medication costs, diagnosis and treatment plan, and understand government regulations including financial and legal information related to admissions.
