Job description Alamo Health Management is looking for an Admissions & Assessments Coordinator to join our team. Ideal candidate will have experience in assessing clients for placement in Assisted Living, Skilled Nursing, Hospital, Hospice, Behavioral Health, and similar environments. We provide transitional housing and resident centered care / wraparound services for clients suffering from mental health, and/or dual diagnosis. manage and direct the admissions process for new clients to our communities. Candidates should be located within an hour's commute to San Ramon with the ability to travel to various locations within the Bay Area, Central California, Northern California, and points south on occasion. This is a remote work position: the office is in our San Ramon Corporate Offices. Responsibilities Conduct initial assessments of potential clients to determine their needs and eligibility for admission. Provide ongoing support to counties, clients and their families throughout the admissions process. Manage the admission process of each new client, ensuring that staff and all departments are prepared for the new admission. Maintain accurate records of all admissions activities. Perform various other duties as needed. Qualifications Must be able clear a California Department of Social Services background check (Livescan) to work in a licensed Adult Residential Facility.
