Patient Advocate | Chronic + Complex Care Connective Wellness - 5.0 Amherst, NH Job Details Part-time $25 - $31 an hour 21 hours ago Qualifications Google Workspace Teamwork HIPAA Customer relationship management Appointment scheduling Full Job Description Patient Advocate Connective Wellness — Amherst, NH (Primarily on-site, with telework flexibility) Part-time, W-2 — starting around 20-25 hours/week, growing toward full time as our diagnostic program does About Connective Wellness At Connective Wellness, we do healthcare differently. We specialize in integrative care for people living with hypermobile Ehlers-Danlos Syndrome (hEDS) and other chronic and complex conditions.
Our mission is simple:
create a place where patients feel truly seen, heard, and supported—because they deserve more than "the runaround." Our team is bold, collaborative, and fueled by compassion. We laugh together, we problem-solve together, and we're building something truly groundbreaking in the world of connective tissue and complex care. What We're Looking For We're looking for a Patient Advocate who wants to make healthcare feel less like a maze and more like a guided journey. Think of yourself as the patient's translator, navigator, and biggest cheerleader. You'll help them cut through medical jargon, understand their options, fight insurance battles, and feel supported every step of the way. A big part of this role is being the front door to our diagnostic program—the calm, human guide who walks patients through what diagnostics involve and stays with them through the whole process, start to finish. This is primarily an on-site role at our Amherst, NH clinic, with the flexibility to telework. We want you here, in the mix—learning the inner workings of the clinic, getting to know our providers and our patients, and being a real part of the team. You'll also research and connect patients with resources across the U.S.—and globally—since our patients come from all over the world. What You'll Do Guide Patients Through Diagnostics Serve as the first point of contact for non-clinical diagnostic intake: welcome patients into the program and orient them to what's ahead. Walk patients through the diagnostic pathway in plain language—what each step is, what to expect, how long it typically takes, and what they'll need to do. Collect and organize intake paperwork, history forms, and required consents, and route anything clinical to the appropriate clinical team member. Hand patients off smoothly to the clinical team for results review, and coordinate that review appointment. Track every patient's status through the diagnostic pipeline so no one falls through the cracks. Support + Educate Patients Break down complex diagnoses and treatment plans into plain language. Help patients understand their rights and resources. Be their voice in conversations with providers. Coordinate Care Schedule and organize appointments, including second opinions. Sit in on virtual visits, take notes, and keep the follow-up train on track. Smoothly guide patients through transitions between providers. Tackle Insurance + Bills Demystify coverage and benefits. Help with claims, appeals, and superbills patients can submit themselves. Spot errors in bills and advocate/negotiate when necessary. Be Their Emotional Anchor Provide a listening ear and encouragement. Connect patients with community resources, support groups, and online networks. Stand up for them when the system makes them feel small. Keep Us Buttoned Up Document accurately and promptly in our systems so the whole team stays in sync. Protect patient privacy and handle all health information in line with HIPAA and our internal policies. Recognize when a patient situation needs clinical attention or escalation, and route it to the right person quickly. A Note on Scope This is a non-clinical role. You are a navigator and advocate—not a provider. You won't diagnose, interpret diagnostic or lab results, or give medical, clinical, or genetic advice. Anything clinical gets routed to our licensed clinical team. (This protects both our patients and you.) What We're Looking For On-site + local: You live in or near Amherst, NH and can work primarily in person at our clinic.
Global mindset:
You're equally comfortable digging into resources far and wide—whether that's a specialist in Boston, a program in California, or research clinics abroad.
Organized under pressure:
You can juggle multiple patients at different stages of a process and never let a detail slip. Experience helps, but heart matters most: We'd love someone with a background in advocacy, healthcare navigation, or medical admin—but if you've lived this (hello, h
EDS/POTS/MCAS
fam), we want to hear from you, too. Certified?
Even better:
A Board Certified Patient Advocate (BCPA) is a plus, but not required if you're eager to grow. Tech-savvy + self-starter: You're comfortable with virtual work tools (our EHR, CRM, and Google Workspace).
People person:
Empathy, patience, and clear communication are your superpowers.
Startup energy:
We're building fast and evolving every day—you thrive in that kind of environment. Pay + Perks Pay range:$25-$29/hour, W-2, commensurate with experience. PTO accrual at 0.5 FTE This role starts at roughly 20-25 hours/week and grows with our diagnostic program—there's a real path to full-time, benefitted employment as patient demand rises. Hours scale with the work, so as the program grows, so do your hours (and your role).
How to Apply Send us:
Your resume. A letter of introduction. It doesn't need to be formal or buttoned-up—honestly, we'd rather it weren't. We want to hear your voice, however that actually sounds. We hire for the things that can't be taught: empathy, compassion, and the ability to truly see and hear people. Skills can be built; that gut-level care can't. So tell us your story—if you know what it feels like to fight for someone, or to be the person who needed someone in their corner, tell us. Tell us why advocacy matters to you. Applications without a letter won't be considered, without exception. We especially encourage applicants who personally live with or understand hEDS and related conditions to apply. Local candidates only (Amherst, NH + surrounding area). Connective Wellness is proud to be an equal opportunity employer.
