Responsibilities Intake & Admissions Receive, review, and process hospice referrals from hospitals, physicians, nursing facilities, home health agencies, and other referral sources. Coordinate and facilitate timely patient admissions in compliance with hospice regulations and company policies. Gather and verify patient demographic, clinical, and insurance information. Communicate admission status and updates to referral sources, patients, families, and clinical staff. Insurance Verification & Authorizations Verify eligibility and benefits for Medicare, Medicaid, managed care, and commercial insurance plans. Obtain and manage prior authorizations and approvals as required by payers. Ensure all insurance documentation is accurate and complete prior to admission. Resolve insurance-related issues, denials, and discrepancies promptly. Maintain current knowledge of Medicare and Medicaid regulations impacting hospice admissions and reimbursement. Documentation & Compliance Maintain accurate and complete patient records within the EMR system. Ensure all admission documentation meets federal, state, and organizational compliance standards. Track referral, admission, and authorization statuses. Assist with audits and quality assurance initiatives. Customer Service & Communication Provide compassionate, professional support to patients and families during the admissions process. Develop and maintain positive relationships with referral sources and healthcare partners. Serve as a liaison between clinical staff, billing, referral sources, and insurance providers. Qualifications High school diploma or equivalent required; Associate's degree preferred. Minimum 2 years of experience in hospice, home health, healthcare intake, admissions, or insurance verification. Strong working knowledge of Medicare, Medicaid, managed care, and commercial insurance plans. Experience obtaining and managing prior authorizations and insurance approvals. Proficiency with EMR systems and Microsoft Office applications. Strong verbal and written communication skills with a professional, compassionate approach. Hospice or home health experience preferred. Knowledge of hospice eligibility criteria and regulatory requirements a plus.
