Job Description

LPN intake coordinator Full-Time Home Health Huntingdon, TN This position entails work primarily in a home health branch office. Some direct patient care is occasionally required. Volunteer Home Care is a locally owned, Medicare-certified home health agency providing skilled nursing, rehabilitation, and personal care services to patients in West Tennessee.
Qualifications:
Basic computer skills, prior experience with EMR a plus Licensed to practice as a LPN in the state of TN Current CPR certification

Essential Functions:
Assist the supervisor with the evaluation of incoming referrals for appropriateness for admission. Obtains verification of benefits/insurance from insurance coordinator, patient demographics and all required clinical support documentation. Reviews the Face to Face documentation and communicates with the supervisor and referral source if any additional documentation is required. Documents the initial order for home health services on each patient. Contact each patient to confirm receipt of referral and schedule admission visit. Confirm demographic information with patient. Report issues with patient to the supervisor. Accurately and completely enter the patient intake information in to the EMR. Assist in relaying messages to field staff, office staff and marketing staff. Ensure complete and appropriate communication is maintained with all members of the care team. Receive verbal orders from physician offices. Document orders received in the EMR. Communicate these orders to field clinicians such as nursing and therapy. Notifies the supervisor as well. Professionally and personally interact with all members of the care team. Demonstrate courteous telephone skills with all internal and external customers. Review staff clinical documentation in the EMR for accuracy, completeness and skill daily. Receive patient lab results and report findings to the appropriate providers. Enter lab results and all orders regarding labs into the EMR. Perform skilled nursing visits as needed. Participate in on-call rotation. Perform other duties as required.
