This role focuses on converting incoming referrals into admissions through timely follow-up, clinical screening, family communication, and paperwork completion — not external sales or community marketing. Key Responsibilities Receive and respond to inbound referrals from hospitals, case managers, discharge planners, and physician offices Follow up promptly by phone with referral sources to gather clinical information and assess appropriateness for admission Coordinate with the Director and clinical team to obtain admission approval Communicate with prospective residents and families to explain services, answer questions, and guide them through the admission process Complete all admission paperwork including financial agreements, consent forms, insurance verification, and preadmission assessments Maintain and update the referral log, tracking conversion rates and referral source activity Coordinate bed assignments and room readiness with nursing and housekeeping Submit required notices and authorizations to insurance/managed care payers Maintain compliance with HIPAA and state/federal admissions regulations Attend morning stand-up or census meetings at both facilities as needed Qualifications 1-3 years of experience in healthcare admissions, case management, or a related role (SNF, assisted living, or hospital setting preferred) Must be strong in CRM management and computer skills Must have strong customer service skills and strong phone communication and follow-through skills Ability to manage referrals across two locations simultaneously Must be outgoing, personable, and empathetic High school diploma required; Associate's or Bachelor's degree preferred
