This position welcomes and orients patients, conducts comprehensive assessments, offers family support, collaborates on individualized care plans, ensures effective care coordination, and develops crisis management and safety plans. Key Responsibilities Welcome new patients and create a smooth transition into our care programs Conduct comprehensive interviews to assess patients' mental and emotional needs Offer guidance, support, and information to foster a collaborative approach to patient care Develop individualized mental health plans with clear therapeutic goals and strategies Coordinate care across providers to ensure comprehensive, ongoing support for patients Develop crisis management and safety plans to protect patient wellbeing during challenging situations Qualifications Bachelor's degree in Social Work, Human Services, Psychology, or a related field Excellent interpersonal and listening abilities Strong verbal and written communication skills Sound problem-solving, decision-making, and critical-thinking abilities Ability to remain calm and composed in high-pressure situations Comfortable collaborating within an interdisciplinary team Experience working with young people and their parents or guardians Empathetic, patient, and understanding approach to care Commitment to maintaining client confidentiality and upholding ethical practice standards Ability to relate effectively with people from diverse lifestyles and cultures Benefits 401(k) retirement plan Employee Assistance Program (EAP) Employee discount on orthodontic treatments Why Join Us If you're passionate about helping others and have experience as an Intake Coordinator or in a similar mental health support role, we'd love for you to join our mission to support and empower young patients and their families.
