  • Full-time Job Summary We are seeking a dynamic and compassionate Outpatient Nurse Practitioner or Physician Assistant to join our vibrant healthcare team.
In this role, you will provide comprehensive outpatient medical care, perform patient assessments, and develop personalized treatment plans. Your expertise will help improve patient outcomes across diverse populations, including pediatrics, geriatrics, and individuals with complex medical needs. This position offers an exciting opportunity to work in a fast-paced environment where your clinical skills and dedication make a real difference in patients' lives. You will collaborate closely with physicians, specialists, and support staff to deliver high-quality outpatient care that emphasizes patient education, preventive health, and chronic disease management. Duties Conduct thorough physical examinations and patient assessments to determine health status and identify medical issues. Develop and implement individualized treatment plans, including medication management, injections such as botulinum toxin treatments, and procedural interventions like suturing or catheterization. Perform diagnostic evaluations utilizing EMR (Electronic Medical Record) systems such as Epic, eClinicalWorks, or Cerner to document findings accurately. Administer injections, IV infusions, dermal fillers, and esthetic laser treatments while adhering to sterile techniques and safety protocols. Manage acute pain cases, triage urgent concerns via telehealth or in-person visits, and coordinate follow-up care for complex cases involving hospital medicine or ICU experience. Educate patients on health maintenance strategies including nutrition, medication adherence, and lifestyle modifications; provide health coaching tailored to individual needs. Collaborate with multidisciplinary teams on discharge planning, case management, and utilization review to optimize patient outcomes across outpatient settings like clinics or assisted living facilities. Ensure compliance with HIPAA regulations and maintain meticulous medical documentation for all patient interactions. Experience Proven experience working in outpatient clinics with a strong background in primary care or specialty areas such as gastroenterology, geriatrics, behavioral health, or occupational health. Hands-on experience with EMR/EHR systems like Epic or Athenahealth for documentation and order entry. Skilled in performing procedures including suturing, phlebotomy, catheterization, spinal taps, and specimen collection/processing. Prior involvement in clinical research or teaching hospital environments is advantageous but not required. Experience managing diverse patient populations including pediatrics (working with children), seniors (senior care), patients with developmental disabilities or dementia (Alzheimer's/dementia care), and those requiring post-acute or hospice & palliative medicine services. Knowledge of CPT coding (Current Procedural Terminology), ICD-9/10 coding standards for accurate billing and documentation. Familiarity with telehealth platforms to deliver remote consultations effectively. Critical care experience such as ICU or Level I/II trauma center exposure enhances the ability to handle complex cases efficiently.
