Licensed Physical Therapist : Outpatient in-home care

BrightStar Care , a homecare agency, is seeking dedicated and compassionate Physical Therapists (PTs) and Occupational Therapists (OTs) to join our dynamic healthcare team. In this vital role, you will deliver expert physiotherapy services directly to patients in their homes, empowering them to regain mobility, reduce pain, and improve overall quality of life. You will be responsible for assessing, planning, and implementing treatment programs to improve patients' mobility and quality of life.

Essential Duties:

Conduct thorough assessments of patients' physical abilities and limitations. Develop individualized care plans based on assessment findings and patient goals. Implement therapeutic interventions including physical therapy techniques, trigger point therapy, and exercise programs. Monitor patient progress and modify treatment plans as necessary. Provide education to patients and their families regarding treatment plans, exercises, and discharge planning. Collaborate with other healthcare professionals to ensure comprehensive patient care. Maintain accurate documentation of patient evaluations, treatments, and progress notes. Stay current with best practices in PT / OT through ongoing professional development.

Pay:

$100-110 per visit (2-hrs) & $125 for evaluations!

Requirements:

PT / OT Virginia State licensure Reliable transportation Active BLS/CPR certification Strong knowledge of anatomy and medical terminology Skilled in managing care for patients across various settings including acute care hospitals, outpatient clinics, and home health environments Proficiency with EMR systems for documentation and communication purposes Strong background in orthopedics, geriatrics, pediatrics, trigger point therapy, and outpatient physiotherapy techniques Excellent communication skills for educating patients and collaborating with healthcare teams Join BrightStar Care in transforming lives through dedicated in-home therapy!
