Job Description
PreferNP partners with eldercare and skilled nursing facilities across Southwest Virginia to bring psychiatric and medical care to residents through onsite telehealth visits. We are hiring a dependable, take-charge Patient Care Liaison to be the in-person presence who makes those visits happen and the trusted link between our patients, the facility, and our providers. This is a hands-on patient care role, not a tech or reception job. You will work directly with elderly residents, many of whom are managing mental health conditions, and you will usually be the only PreferNP team member onsite. That means this role is built for someone who works independently, takes charge, and figures problems out in the moment. If you are the kind of person who walks into a busy situation, sizes it up, and handles it without waiting to be told what to do, you will do well here. If you need someone standing over your shoulder, this is not the role for you. What you'll do Greet and build rapport with elderly residents and be a familiar, reassuring presence during their visits Escort patients to their appointments and stay present during psychiatric visits as a supportive face Set up and manage the telehealth device (iPad) for each visit and troubleshoot simple tech issues on your own Coordinate with facility nursing and administrative staff and relay patient updates accurately to our providers Schedule and track patient visits on each resident's care cadence, and follow up on missed appointments Take ownership of the day onsite: solve the small problems that come up, keep visits on schedule, and keep things running What we're looking for Reliability above all. Our patients and providers depend on you showing up. This role is only a fit for someone who treats being there, on time, as non-negotiable. A go-getter who works independently. You take initiative, take charge of the room, and figure things out as they happen instead of waiting for direction.
Clinical or patient-care experience:
Medical Assistant (MA, RMA, CMA, CCMA), CNA, LPN, patient access, or medical front desk. Hands-on experience in a real clinical setting is strongly preferred. Composure under pressure. You stay calm and steady when a day gets busy or unpredictable. Warmth and patience with elderly patients, and comfort around mental health conditions Comfort with basic technology (tablets, video visits) Reliable transportation and the ability to travel between nearby facilities Local to Rural Retreat / Abingdon area. Pay, raises, and guaranteed hours Start at $20/hour Earn a raise after 90 days of dependable attendance Guaranteed 20 hours per week after 30 days of dependable attendance, so you can count on steady income Additional hours and first opportunity at new facilities for reliable liaisons who cover more than one location (Rural Retreat and Abingdon are about 30 to 35 minutes apart) What we provide: We'll get you set up on our EHR and walk you through the basics. From there, this is a learn-as-you-go role. You'll have support by phone when you need it, but you'll be expected to get comfortable quickly and run the day on your own.
