Intake Coordinator
Position Qualifications:
The Intake Coordinator shall have some college credits in the following areas degree in social work, counseling, sociology, psychology, business/public administration or management, and be able to qualify as a QPP within the first 180 day of hire. Intake coordinator must have plus five years of experience in an admin and service program including five years in an administrative or supervisory capacity. Excellent written and verbal skills are required. The ability to recruit is also required. Scope of services
- Establishing and maintaining contact with new clients and caretakers.
Explaining and collaborating services with other agencies and potential clients. Make referrals as needed to other agencies.
Position Responsibilities:
Make initial contact with referrals Maintain contact with referral source Maintain a neat and organized office at all times Set Clients up for services (following intake steps below)
- Screen Clients and evaluate if criteria for programs are met Make referrals to PCP/ Psychiatrist/ Outpatient Therapy/ Dreams of Hope Foundation Communicate with counselors regarding assessments/ authorizations/ and reports that are due.
Organize and maintain new client documentation Complete all initial and annual intakes Assist all new referrals until they are assigned to Case worker Prepare assessments
- and SRA documentation Send off Releases of information as needed Communicate with assessor and schedule assessments Track/ sendoff authorizations to insurance companies Maintain communication with MCOs Maintain and update agency trackers as needed Ensure Medicaid is checked Monthly
- communicate with clients and workers regarding Medicaid status Input and Maintain Information for HSCSS Service Authorizations
- Complete Setup forms after every assessment Assist with assign clients to workers Prepare management report Weekly Conduct and facilitate New Hire Training (as needed) Answer phones as needed Assist with HR as needed Required to carry at least one MHSS case for position Assist with covering cases as needed Assist with DOH as needed
- Performs other duties as assigned.
Attend and participate in new employee orientation, in-service training, conferences as scheduled and performs other duties as assigned. Receive 40 hours of training per year, maintain and keep current all first aid and CPR training requirements. I understand that by taking this position I am required to be drug free and will not be intoxicated while at work. I understand that I may be drug tested at anytime.
