Job Description

Position Summary Faithworks, Inc. is seeking a compassionate and detail-oriented Intake Coordinator to serve as the first point of contact for individuals and families seeking assistance through the Pathways Program. This position plays a critical role in creating a welcoming, respectful, and trauma-informed experience for participants while gathering information needed to connect them with appropriate services and support. The ideal candidate possesses strong customer service skills, excellent attention to detail, and a commitment to treating all participants with dignity and compassion. This position serves as the gateway to services and helps ensure participants receive timely support during periods of crisis and transition. Essential Duties and Responsibilities Serves as the primary point of contact for individuals seeking assistance through the Pathways Program. Responds to inquiries received through phone calls, email, online submissions, community partners, and walk-in visits. Conducts initial screenings, assesses urgency, collects participant information, and obtains required consents Gathers demographic, household, and program eligibility information accurately and efficiently. Explains program services, eligibility requirements, documentation needs, and program limitations. Obtains participant consent forms and required releases of information. • Coordinates referrals to the Resource Navigator for further assessment and assistance. Documentation and Data Management Creates and maintains participant records in the program database. Enters accurate and timely data related to inquiries, referrals, and participant interactions. Maintains call logs, intake records, and supporting documentation. Supports quarterly reporting and data collection efforts by maintaining complete and accurate records. Program Support Assists with participant communications and appointment reminders. Participates in team meetings and program training activities. Supports outreach events and community engagement efforts as needed. Assist with administrative tasks that support efficient program operations. Required Knowledge, Skills, and Abilities Excellent customer service and interpersonal communication skills. Strong active listening skills and the ability to communicate with empathy and professionalism. Ability to remain calm and effective when working with individuals experiencing crisis or emotional distress. Basic understanding of community resources and human service systems. Ability to follow established procedures while exercising sound judgment. Proficiency with Microsoft Office and database systems. Minimum Qualifications High school diploma or equivalent. One year of experience in customer service, administrative support, nonprofit services, human services, call center operations, church/community ministry, healthcare reception, or a related field. Equivalent combinations of professional and lived experience may be considered. Preferred Qualifications Bilingual proficiency in English and Spanish.
