JOB SUMMARY
The Health Information and Admissions Coordinator position supports front-desk operations, admissions coordination, and the management of resident medical records in a Skilled Nursing and Long-Term Care facility. This role ensures timely, compliant admissions, survey readiness, and confidentiality while serving as the first point of contact for residents, families, and referral partners. Participates in Payroll-Based Journal (PBJ) data submission in accordance with CMS requirements. Strict compliance with
HIPAA, CMS
regulations, and facility policies is required at all times. Confidentiality and professionalism are essential.
ESSENTIAL RESPONSIBILITIES
Front Office Operations & Patient Access Greet and assist residents, families, visitors, referral partners, vendors, and surveyors Answer and route incoming telephone calls Maintain visitor logs and follow safety protocols Coordinate communication with admissions, nursing, therapy, and administration Manage mail, deliveries, and front desk organization Admissions Coordination Support Responsibilities Assist Admissions Coordinator with pre-admission and admission workflows Assemble and prepare admission packets and charts Enter demographic and insurance data into PointClickCare Track and follow up on pending admission documentation Support resident and family intake during admission Communicate admission status to interdisciplinary teams Health Information Management Responsibilities Maintain and secure medical records per HIPAA and CMS regulations Scan, upload, and index medical documents Track record completeness and required signatures Support surveys, audits, and regulatory reviews
POSITION QUALIFICATIONS
- Education, Experience and Licens e High school diploma or equivalent required Central Registry, required. HIM coursework or certification preferred SNF/LTC, admissions, or medical records experience preferred. Additional Skills Ability to effectively communicate in English, both reading and writing. Multi-line telephone knowledge. Computer knowledge. Basic clerical training, including basic filing procedures (both numerically and alphabetically). Ability to function independently with minimal direction.
