Position Overview The Respiratory Therapist will provide on‑site respiratory assessments and monitoring for post‑hospitalization patients residing in Skilled Nursing Facilities. This role focuses on early identification of changes in patient condition, timely intervention, and reducing avoidable hospital readmissions. Strong independent judgment and collaborative communication with facility staff are essential.
