The SLP collaborates closely with physicians, nursing staff, rehabilitation professionals, residents, and families to promote optimal outcomes. Essential Duties & Responsibilities Evaluate residents and develop individualized treatment plans Provide speech, language, cognitive, communication, and swallowing therapy services Assess and manage dysphagia and recommend appropriate dietary modifications Maintain accurate and timely clinical documentation Collaborate with interdisciplinary team members to coordinate resident care Educate residents, families, and caregivers regarding treatment goals and progress Participate in care planning meetings and discharge planning as needed Ensure compliance with all regulatory, ethical, and facility standards Qualifications Current California Speech-Language Pathologist license required Certificate of Clinical Competence (CCC-SLP) preferred Current CPR certification preferred Strong clinical assessment and treatment skills Excellent communication and interpersonal abilities Skilled nursing, post-acute, or rehabilitation experience preferred Ability to work independently and collaboratively within a multidisciplinary team Equal Opportunity Employer Concord Post-Acute is an Equal Opportunity Employer and is committed to creating an inclusive workplace for all employees.
