We serve medically complex older adults and individuals with disabilities in a structured, community-based setting. Our interdisciplinary team is committed to delivering high-quality, person-centered care that supports health, function, and dignity. Position Summary We are seeking a licensed Speech-Language Pathologist (SLP) on a contract or per diem basis to conduct formal swallowing evaluations for participants at risk of dysphagia and aspiration. This role is a key component of our clinical safety protocol and operates in alignment with California Title 22 CBAS regulatory requirements. Key Responsibilities Conduct clinical swallow evaluations (CSE/bedside) for participants identified as aspiration risk Document findings in compliance with California Title 22 CBAS standards Collaborate with the program's Medical Director or attending physician for co-signature on instrumental findings and physician referrals as required Provide diet and texture modification recommendations (IDDSI framework preferred) Communicate findings and care recommendations to the interdisciplinary team Requirements Active California SLP license in good standing (Speech-Language Pathology & Audiology & Hearing Dispensers Board)
ASHA CCC-SLP
certification preferred Experience with dysphagia evaluation in geriatric, SNF, or
ADHC/CBAS
populations strongly preferred Familiarity with California Title 22 regulations a plus Ability to work collaboratively within an interdisciplinary care team Schedule & Compensation Contract / per diem Monday - Friday, business hours: 8:00 AM - 3:00 PM (flexible scheduling within these hours) On-site at DayOut
