Speech Therapist (SLP)
  • Full-Time Sequoia Transitional Care
  • Porterville, CA Pay range: $60
  • $65/hour (DOE) General Purpose The Speech-Language Pathologist (SLP) evaluates and treats patients, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical record.
The SLP also conducts in-services and provides ongoing training for facility staff to ensure the highest quality of care and rehabilitation outcomes. Essential Duties & Responsibilities Evaluate patients within twenty-four (24) hours or one business day of physician referral; document any cause for delay in the medical record. Develop effective treatment plans and obtain approval for services from the referring physician. Provide therapy services according to the physician's treatment plan. Supervise Speech-Language Pathologist Assistants in direct patient care and related activities in accordance with state practice acts. Communicate with supervisors, physicians, and interdisciplinary team members regarding patient progress, problems, and care plans. Participate in Patient Care Conferences, Utilization Reviews, and Rehabilitation Team meetings. Conduct in-service training programs for facility staff as needed. Maintain accurate and timely documentation, including evaluations, daily treatment notes, weekly progress reports, recertifications, and discharge summaries. Educate patients, families, and nursing staff on maintenance programs and caregiver techniques in preparation for discharge. Participate in discharge planning and interdisciplinary care coordination. Serve as a clinical preceptor for CFY/RFP Speech-Language Pathologists and student programs. Ensure compliance with all applicable state practice acts, federal regulations, and facility policies. Exhibit professionalism, empathy, and effective communication with patients, residents, families, and colleagues. Supervisory Responsibilities Supervise Clinical Fellows (CFY) and assist nursing in the training of Restorative Aides. Qualifications Master's or Doctoral degree in Speech-Language Pathology from an accredited program. Current, active California Speech-Language Pathologist license in good standing. Proficient in clinical documentation systems such as Casamba and PointClickCare (PCC). Must meet continuing education requirements per state practice regulations. Strong communication, organizational, and clinical skills. Ability to work effectively as part of an interdisciplinary rehabilitation team. Physical Requirements Frequent standing, walking, pushing, and pulling. Occasional lifting of up to 100 lbs with assistance. Occasional stooping, kneeling, and reaching. Regular talking, hearing, and close visual attention required.
