Home Health Speech-Language Pathologist Bellestar Home Health - 5.0 Sacramento, CA Job Details Per diem $65 - $70 an hour 1 day ago Benefits Opportunities for advancement Qualifications Spanish CPR Certification Master's degree in speech-language pathology (SLP) Medicare BLS Certification Master's degree Driver's License Basic life support Speech-Language Pathology License Home health Full Job Description Bellestar Home Health is seeking a dedicated and compassionate Speech-Language Pathologist (SLP) to provide in-home therapy services to patients with communication, cognitive, and swallowing disorders. This role focuses on improving patients' quality of life through individualized, patient-centered care in the home setting. Key Responsibilities Evaluate and assess patients with speech, language, cognitive, and swallowing disorders Develop and implement individualized treatment plans in accordance with physician orders Provide therapy to improve communication, cognition, and safe swallowing function Perform dysphagia assessments and recommend appropriate diet modifications Educate patients, families, and caregivers on therapy techniques and safety strategies Document all evaluations, visits, and progress notes in compliance with CMS guidelines Collaborate with interdisciplinary team members (RN, PT, OT, MSW, physicians) Participate in case conferences and care coordination Ensure compliance with all federal, state, and agency regulations Qualifications Master's degree in Speech-Language Pathology Active California SLP license Certificate of Clinical Competence (CCC-SLP) preferred Minimum 1 year of experience (Home Health preferred) Knowledge of Medicare/Medi-Cal home health guidelines Strong clinical, communication, and documentation skills Valid driver's license, reliable transportation, and auto insurance Current CPR certification (BLS) Preferred Qualifications Experience with dysphagia management and modified diets Familiarity with EMR systems (e.g., Finale Health or similar) Bilingual (Spanish or other languages) is a plus Why Join Bellestar Home Health?
