About the Role Rx HealthCare Services is seeking a compassionate and motivated Speech Therapist / Speech-Language Pathologist to join our Home Health team in either a part-time or full-time role . This is a great opportunity for a Speech Therapist who is passionate about helping patients improve their communication, swallowing, and overall quality of life in the comfort of their own homes. Home health allows clinicians to provide meaningful, one-on-one care while building strong relationships with patients and families. Prior home health experience is preferred but not required. To learn more about our company, visit: www.rxnursing.com Responsibilities The Speech Therapist / Speech-Language Pathologist will provide skilled speech therapy services to patients in the home health setting.
Duties may include:
Evaluating patients with speech, language, cognitive-communication, voice, and swallowing difficulties Assessing and treating swallowing disorders and communication difficulties Developing and implementing individualized treatment plans based on patient needs and physician orders Monitoring patient progress and modifying treatment plans as appropriate Educating patients, caregivers, and family members on therapy goals, safety, and home programs Collaborating with the interdisciplinary home health team to support patient-centered care Completing timely and accurate clinical documentation Communicating changes in patient condition or needs to the appropriate team members Requirements Current and valid California Speech-Language Pathologist license Master's or Doctoral degree in Speech-Language Pathology Strong communication, organization, and clinical judgment skills Ability to work independently in the field while collaborating with the care team Reliable transportation, valid driver's license, and proof of auto insurance Home health experience preferred, but not required Work Environment This is a field-based home health position. The Speech Therapist will travel by automobile to patient homes and may work in a variety of residential settings. The role may involve exposure to changing weather conditions and varying home environments based on assigned caseload.
