(After this we can mention job qualifications ) Job Duties Assess patients' functional cognitive, speech and swallowing abilities and develop individualized care plans aligned with their goals and medical needs Implement therapeutic interventions in a senior living and in-home setting Collaborate with multidisciplinary teams including occupational therapists, physical therapists, physicians, and caregivers Manage case documentation accurately and maintain detailed patient records in compliance with healthcare regulations Educate patients and families about treatment goals, progress, and strategies for continued improvement Participate in ongoing professional development activities to stay current with best practices in speech therapy Requirements Valid speech therapy license in the state of California Experience working in outpatient clinics; additional experience in inpatient, skilled nursing facilities, home health, or school-based settings is preferred Knowledge of engaging patients with cognitive deficits Familiarity with working with diverse populations including geriatrics, individuals with autism or disabilities Strong understanding of physiology, anatomy, medical terminology, and patient care principles Excellent communication skills for collaborating with healthcare teams and educating patients and families Compassionate attitude with a commitment to improving patient outcomes and quality of life This position provides an excellent opportunity for professional growth within a supportive healthcare environment.
