• Part-time, Full-time, Contract, Internship We are currently seeking providers for Early Intervention in Larimer, Weld and Jefferson County in disciplines of Speech therapy, Physical Therapy and Occupational Therapy supporting clients in Larimer and Weld counties as well as at LightHouse WholeHealth Clinic.

All under one roof, therapists will be able to collaborate using a whole child approach for treating children. This is a unique setting where families can access support in all areas including mental health services which is often a needed, yet, lacking part of a developmental therapy clinic. Job Description:

Provide superb in-home Early Intervention services to families following a coaching model for caregivers. Responsible for following Early Intervention Colorado guidelines for completing and attending scheduled IFSP meetings. Responsible for completing outcome progress updates and annual assessments when needed. Participate with LightHouse WholeHealth clinical team for ongoing support. A minimum of 20 billed hours per week for 1099 Contractors Appropriately communicate information and observations regarding the patient's progress, goals, etc…in writing and/or verbally to the parents (via evaluation reports, daily notes, home program suggestions). Documents daily/weekly therapy for each patient. Maintains a good working relationship with co-workers. Performs other duties as assigned or required. Education, Knowledge and Skills Required Master's Degree in Speech Language Pathology, Physical or Occupational Therapy Pediatric Experience Embody a playful spirit and be able to connect and engage with young children and their families. Timely completion of evaluation reports, treatment plans, therapy notes and discharge reports following state Early Intervention requirements. Strong collaboration skills with therapists to promote multidisciplinary model of treatment. Strong parent relationships and willingness to coach parents on all aspects of treatment being provided. Responsible for carrying own Professional Liability, HIPPA and auto insurance policies.
