Position Summary The Speech-Language Pathologist (SLP) provides skilled speech therapy services to patients in their homes in accordance with physician orders, the plan of care, agency policies, and applicable state and federal regulations. The SLP evaluates communication, cognitive, and swallowing disorders, develops and implements therapy interventions, and works collaboratively with the interdisciplinary care team to improve patient safety, independence, and quality of life. Reporting Structure This position reports to the Director of Nursing (DON), Clinical Manager, or designated clinical leadership. The SLP functions as part of the interdisciplinary care team and collaborates with nursing, therapy, and physician teams to support coordinated patient care. Scope of Practice The Speech-Language Pathologist performs services within the scope of Florida SLP licensure, Medicare Conditions of Participation, physician orders, and agency policies. The SLP is responsible for evaluation, treatment planning, reassessment, discharge planning, and therapy services within the home health setting. Essential Job Functions 1. Patient Evaluation & Care Planning Perform initial and ongoing patient evaluations to assess speech, language, cognitive, voice, and swallowing disorders Complete comprehensive assessments and participate in OASIS-related coordination as applicable Develop, implement, and update individualized speech therapy treatment plans Ensure physician orders are appropriate and aligned with the patient's therapy needs 2. Skilled Therapy Services Provide skilled speech therapy interventions in accordance with physician orders and established care plans Treat speech, language, communication, cognitive, and swallowing impairments Support safe swallowing function and dysphagia management in accordance with clinical standards Promote restoration of communication abilities, cognitive function, and safe oral intake in the home setting 3. Patient & Family Education Teach and counsel patients and caregivers regarding therapy goals, communication strategies, swallowing precautions, and cognitive support techniques Evaluate the home environment and identify strategies to improve safety and functional communication Coordinate additional services or referrals when appropriate to support patient outcomes 4. Documentation & Compliance Complete timely and accurate clinical documentation, progress notes, reassessments, and discharge summaries Evaluate effectiveness of therapy interventions and update the plan of care as needed Maintain compliance with Medicare requirements, state regulations, and agency policies 5. Team Collaboration Communicate regularly with physicians, nurses, therapists, and interdisciplinary team members regarding patient progress and needs Participate in care conferences, discharge planning, staff meetings, and required in-services Support continuity of care through coordinated interdisciplinary communication Qualifications Master's degree in Speech-Language Pathology from an accredited program required. Current state Speech-Language Pathologist (SLP) licensure in good standing required. Minimum of one (1) year of clinical experience in speech therapy required; home health, acute care, or rehabilitation experience preferred. Certificate of Clinical Competence (CCC-SLP) preferred, if applicable Current CPR certification required Valid driver's license, reliable transportation, and auto insurance Ability to pass background screening and all compliance requirements Skills We Value Strong clinical judgment and therapy assessment skills Excellent communication and patient education abilities Compassionate, patient-centered approach to care Ability to work independently in a home health setting Strong documentation and organizational skills Commitment to compliance, safety, and quality outcomes Physical Requirements Frequent travel to patient homes within assigned territory Ability to stand, walk, bend, lift, and assist patients as needed Ability to perform therapy assessments and interventions in home environments Ability to use electronic documentation systems and therapy equipment Ability to communicate effectively with patients, families, and care team members Work is performed in patient homes and community settings with exposure to variable environmental conditions, including pets, smoke, infectious diseases, and weather-related travel conditions.
