Job Description:
Observes and records activities and findings in the clinical record, reporting patient reactions and changes to the physician or primary Case Manager. Performs evaluations to determine diagnosis, prognosis, and plan of care in consultation with the physician. Determines specific interventions, duration, and frequency needed to achieve anticipated goals and outcomes. Instructs other health team personnel in phases of speech therapy relevant to patient care. Selects, applies, or modifies skilled interventions consistent with state practice acts to achieve the best patient outcomes. Assesses patient responses to interventions and perform reassessments as required. Documents treatment provided in medical records, updating care plans as indicated. Participates in clinical record reviews, agency staff meetings, therapy meetings, and other activities as requested. Provides supervision and guidance to aides, ensuring compliance with care plans and documentation requirements. Instructs patients and caregivers on speech language improvement programs, including adaptive equipment and home programs. Other duties as assigned.
Knowledge, Skills, and Abilities:
A working knowledge of community resources and an ability to refer patients and families appropriately. [Required] Knowledge of Home Care Regulations and Third-Party Reimbursement as it impacts care delivery. [Required] Entry level competency of occupational therapy practice, theory, principle, and applications as defined in competency department policies. {Required] Basic computer skills and knowledge of Microsoft Office. {Required] Must be able to read, write, and speak conversational English. [Required]
