Qualifications & Preferred Experience:
Master's degree in Speech-Language Pathology Valid, current state licensure as an SLP At least one year of experience in home health or healthcare SLP roles Proficient in conducting assessments, crafting care plans, and documenting progress Strong interpersonal, communication, and organizational skills Ability to work independently while collaborating with multidisciplinary teams Reliable transportation and willingness to travel locally for home visits
Role Overview & Core Responsibilities:
Conduct comprehensive speech, language, and swallowing evaluations for patients in their homes Develop and implement individualized treatment plans addressing communication, speech, voice, and feeding/swallowing disorders Educate patients, caregivers, and family members on therapeutic techniques and progress Coordinate care with physicians, nurses, occupational therapists, and other healthcare professionals Maintain accurate, timely documentation consistent with regulatory and agency standards Monitor, reassess, and adapt interventions based on patient progress and goals Model compassionate, evidence-based care in every encounter
