Job Description

Category Homehealth - SLP Job Type Contract Make a meaningful difference in the lives of patients by delivering Speech-Language Pathology services in a supportive home health setting. This contract opportunity enables you to work closely with patients and families, helping individuals achieve greater communication and swallowing function in the comfort of their own homes.
Key Qualifications:
Master's degree in Speech-Language Pathology from an accredited program Current SLP license Prior experience in home health or healthcare settings preferred Strong communication, organization, and documentation skills Ability to develop customized care plans and work independently with minimal supervision
Responsibilities:
Conduct comprehensive assessments of speech, language, cognitive-communication, and swallowing disorders Develop and implement individualized therapy treatment plans based on patient assessments Provide education and training to patients, caregivers, and family members to improve at-home care Maintain accurate clinical documentation in compliance with healthcare regulations Collaborate with referring physicians, nurses, and other healthcare professionals to ensure coordinated patient care Monitor, evaluate, and document patient progress, adapting plans as needed for optimal outcomes
