Job Description

Category Homehealth - SLP Job Type Contract Provide compassionate, evidence-based care in a home health setting, working independently with patients in their own environments. As a Speech-Language Pathologist specializing in home health services, you will assess, diagnose, and design treatment plans to support patients with communication and swallowing disorders. This contract position offers a unique opportunity to make a meaningful impact by helping patients achieve their highest level of functional communication and quality of life.
Key Qualifications & Experience:
Valid and current Speech-Language Pathologist license Prior experience in home health or similar healthcare setting strongly preferred Expertise in evaluating and treating a range of speech, language, voice, and swallowing difficulties Strong interpersonal skills for effectively communicating with patients, families, and the care team Ability to work autonomously, demonstrating reliability and initiative Strong documentation skills are essential

Role Responsibilities:
Conduct comprehensive assessments of patients in their homes Develop and implement individualized treatment plans targeting communication, cognitive, and swallowing deficits Educate and support families and caregivers regarding treatment approaches and at-home strategies Collaborate with physicians, nurses, and other allied health professionals to ensure coordinated patient care Maintain accurate and timely patient records in compliance with professional and regulatory standards Monitor progress and adjust interventions as needed to achieve patient goals
