Job Description

Category Homehealth - SLP Job Type Contract Make a positive impact by providing high-quality speech-language pathology services to patients in a home health setting. This contract opportunity offers the chance to collaborate with interdisciplinary teams, promote improved patient outcomes, and support individuals in achieving greater communication and swallowing abilities in the comfort of their homes.
Key Qualifications:
Licensed Speech-Language Pathologist (SLP) Previous experience in home health or similar healthcare environments preferred Strong clinical assessment and documentation skills Ability to create and implement individualized care plans Excellent communication and interpersonal skills Commitment to delivering compassionate, patient-centered care
Primary Responsibilities:
Conduct thorough speech, language, cognitive, and swallowing evaluations for home-based patients Develop and execute evidence-based treatment plans tailored to each patient's needs and goals Document all visits and progress in accordance with regulatory and organizational standards Educate patients, caregivers, and families on communication strategies and therapeutic techniques Collaborate regularly with physicians, nurses, and other allied health professionals to coordinate comprehensive care Monitor patient progress and modify care plans as required to achieve optimal outcomes
