Job Description

Category Homehealth - SLP Job Type Contract Seeking an experienced Speech-Language Pathologist to provide exceptional care in a home health setting. This contract opportunity focuses on delivering tailored speech therapy services to patients in their own homes, helping them achieve the best possible outcomes in communication and swallowing functions.
Qualifications:
Active, valid Speech-Language Pathology license Graduate of an accredited SLP program (Master's degree required) Minimum of one year SLP experience, preferably in home health or healthcare Understanding of home care regulatory requirements and best practices Strong interpersonal, communication, and organizational skills Ability to work independently and manage a caseload Valid driver's license and reliable transportation
Schedule/Hours:
Flexible daytime schedule (exact hours discussed during interview)
Responsibilities:
Conduct speech, language, cognitive, communication, and swallowing assessments in patients' homes Develop, implement, and modify individualized treatment plans based on patient needs and progress Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care Document patient progress, update records, and communicate with relevant professionals Provide education, strategies, and support to patients and families to empower ongoing management of speech/language disorders Ensure compliance with all healthcare regulations and documentation standards Maintain regular communication with agency and home health coordinators for efficient workflow and scheduling This contract position offers the chance to make a meaningful impact on patients' quality of life, increasing independence and function where it matters most—their home.
