Your responsibilities will include:
Develop Individualized Care Plans Evaluate patient needs and create personalized speech therapy treatment plans. Modify treatment plans based on patient progress and changing clinical needs. Utilize therapeutic, cognitive, communication, swallowing, and self-care interventions to maximize patient independence. Improve Patient Outcomes Deliver evidence-based therapy focused on: Speech and language Swallowing disorders Cognitive function Communication skills Breathing and voice rehabilitation Educate patients on home exercise programs that support continued progress. Reduce Hospital Readmissions Teach patients and caregivers preventative strategies that promote safety, recovery, and independence. Encourage healthy decision-making to improve long-term outcomes. Educate and Support Families Provide education and counseling regarding diagnoses, treatment plans, and disease management. Empower caregivers with the knowledge and tools needed to support patients throughout their therapy journey. Maintain Clinical Excellence Complete timely and accurate clinical documentation in accordance with Medicare guidelines and agency policies. Communicate patient progress with physicians and interdisciplinary team members. Participate in required meetings, ongoing education, and in-service training. What We're Looking For We're seeking a dedicated clinician who is: A compassionate and patient-focused communicator Detail-oriented and organized Collaborative with physicians, patients, caregivers, and fellow clinicians Passionate about improving lives through exceptional home health care Qualifications Current Speech Language Pathologist license in the state of Kentucky. Minimum of one year of Speech Therapy experience in an acute care setting. Home health experience preferred. Valid driver's license, reliable transportation, and current auto insurance. Ability to meet the physical demands of home health practice.
