Speech-Language Pathologist (SLP)/Speech Therapist

Job description Accelerated Home Health Care is seeking a licensed and compassionate Speech-Language Pathologist (SLP) to provide in-home therapy services under physician orders and in coordination with the care team.

Key Responsibilities:

Conduct evaluations and develop individualized treatment plans. Implement evidence-based speech and language interventions. Educate patients and caregivers on strategies and exercises. Document patient progress and communicate with the interdisciplinary team. Follow physician orders and all applicable regulations.

Minimum Certification & Licensing Requirements:

Master's degree in Speech-Language Pathology. CCC-SLP certification and state licensure. Completion of agency onboarding/credentialing. Up-to-date TB test and reliable transportation.

Preferred Qualifications:

Home health experience preferred. Strong communication and clinical skills. Ability to work independently in a home setting.

Working Conditions:

Travel to patient homes required. Flexible scheduling including occasional weekends. Physical demands include standing, bending, and assisting patients. today to make a difference at home.
