Contract | Local or Traveler Position Overview We are seeking a compassionate and skilled Speech-Language Pathologist (SLP) to provide speech therapy services in a home health setting. The SLP will evaluate, diagnose, and treat patients with speech, language, cognitive-communication, voice, and swallowing disorders while developing individualized treatment plans to improve patient outcomes and quality of life. The ideal candidate will have prior home health experience, strong clinical assessment skills, and the ability to work independently while collaborating with interdisciplinary healthcare teams. Responsibilities Evaluate, diagnose, and treat speech, language, cognitive-communication, voice, and swallowing disorders Develop individualized treatment plans based on patient needs and goals Provide therapeutic interventions and monitor patient progress Educate patients and families on treatment strategies and home exercise programs Collaborate with physicians, nurses, caregivers, and healthcare professionals to coordinate care Maintain accurate and timely documentation in EMR systems Ensure compliance with Medicare guidelines, home health regulations, and professional standards Participate in care planning and interdisciplinary team meetings Promote patient independence, safety, and quality of life Adjust treatment plans based on patient response and clinical progress Requirements Master's Degree in Speech-Language Pathology from an accredited program Active New Jersey Speech-Language Pathologist (SLP) License Minimum 1 Year of Speech Therapy Experience Home Health Experience Required Current BLS/CPR Certification Strong assessment, treatment planning, and documentation skills Ability to work independently in a home health environment Excellent communication and interpersonal skills Knowledge of Medicare regulations and home health standards Preferred CCC-SLP (Certificate of Clinical Competence) from ASHA Experience with EMR systems Experience treating adult and geriatric patient populations Strong interdisciplinary collaboration skills For more details contact at or Call / Text at 516-862-1169.
