Full Job Description Overview:
We are seeking a compassionate and dedicated Speech Therapist (Speech-Language Pathologist) to join our home health team. The Speech Therapist will provide in-home evaluation and treatment services to patients experiencing speech, language, communication, cognitive, or swallowing disorders. The ideal candidate is patient-focused, organized, and passionate about helping patients improve independence and quality of life in a home care setting.
Pay:
TBD Responsibilities:
Evaluate patients with speech, language, cognitive, voice, and swallowing disorders. Develop individualized treatment plans based on patient needs and goals. Provide speech and swallowing therapy in patients' homes. Educate patients and caregivers on communication strategies and home exercises. Monitor patient progress and adjust treatment plans as needed. Maintain accurate documentation and clinical records. Collaborate with physicians, nurses, therapists, and caregivers. Ensure compliance with Medicare, home health, and healthcare regulations. Travel to patient homes within assigned service areas.
Qualifications:
Master's degree in Speech-Language Pathology. Current Nrvada Speech-Language Pathologist (SLP) license. Certificate of Clinical Competence (CCC-SLP) preferred. Home health experience required. Current CPR/BLS certification. Valid driver's license and reliable transportation. Strong communication and organizational skills.
