Gurwin Healthcare System is a full healthcare continuum providing extraordinary care to the Long Island community and beyond. Set primarily on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, Gurwin's Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens), Island Nursing and Rehab Center, and two certified (CHHA) and licensed home care (LHCSA) agencies. We are currently seeking a per diem Speech Language Pathologist who will embody the passion and commitment required to deliver the ultimate care for our residents.EDUCATION AND EXPERIENCE REQUIREMENTS
ASHA Certificate of Clinical Competency. Two years' experience in Speech Language Pathology, one year geriatric related experience. New York State License or eligibility and current certification. Trach/Vent experience preferred. Nethealth experience preferred.DUTIES AND RESPONSIBILITIES
Performs screens, evaluations and provides treatments as prescribed by the attending physician. Recognition of the relative and respective physical, psychosocial and emotional changes that occur at different stages in life, the understanding of which promotes the ability to provide quality care. Demonstrates the ability to discern the needs and behaviors of subacute/rehab patients appropriately, as they differ from those of the elderly. Addresses/accommodates the age-specific physical, psychosocial and emotional changes in residents/patients when planning programs and/or delivering care. Communicates clearly and comprehensibly to residents, patients and family members, understanding changes in cognition and allowing sufficient time for the processing of information. Evaluates and treats residents under physician order. Maintains appropriate record keeping in accordance with facility and regulatory guidelines. Consults with the resident, family members, attending physician and other interdisciplinary team members concerning the treatment and goals for each resident. Acts as Clinical Instructor, providing education and supervision to SLP students. Is knowledgeable of current regulatory requirements and assists in maintaining compliance within the department. Assists in the revision of clinical and administrative policies and procedures.Similar remote jobs
