Health, life, and disability insurance Paid Time Off (PTO) Mileage reimbursement Key Responsibilities Provide skilled speech therapy services based on physician orders and individualized care plans Conduct comprehensive evaluations to assess speech, language, voice, cognition, and swallowing abilities Develop and implement targeted treatment programs to improve communication and functional independence Educate patients and caregivers on therapeutic techniques, safety, and home-based exercises Collaborate with nurses, physical therapists, occupational therapists, social workers, and the interdisciplinary care team Document all evaluations, treatments, progress notes, and care plans accurately in the EMR system Participate in care planning, case conferences, and team communication as needed Deliver compassionate, patient-centered therapy that supports optimal outcomes Qualifications Active Speech-Language Pathologist (SLP) license in Ohio Certificate of Clinical Competence (CCC-SLP) preferred Minimum of 1-2 years of clinical experience (home health experience strongly preferred) Knowledge of EMR documentation and home health regulations preferred Strong communication, clinical reasoning, and patient education skills Current CPR certification Reliable transportation, valid driver's license, and active auto insurance Work Environment Travel required to patient homes within the service area Must meet physical requirements, including occasional lifting, bending, and prolonged walking or standing If you are a dedicated SLP who values flexibility, autonomy, and patient-centered care, we would love to hear from you.
