Are you a passionate Speech-Language Pathologist looking for a dynamic and rewarding career? We are seeking a dedicated SLP to provide high-quality, patient-centered care across Skilled Nursing Facilities, Home Health, and Mobile Outpatient settings. This unique opportunity offers clinical variety, scheduling flexibility, and the ability to make a meaningful impact in the lives of older adults and medically complex patients. Position Summary The Speech-Language Pathologist evaluates, develops, and implements individualized treatment plans to address communication, cognitive-linguistic, voice, and swallowing disorders. The SLP collaborates with interdisciplinary team members to maximize patient outcomes and promote independence and quality of life. Responsibilities Conduct comprehensive speech, language, cognitive, and swallowing evaluations. Develop individualized treatment plans based on patient needs and goals. Provide skilled therapeutic interventions for communication, cognition, voice, and dysphagia disorders. Complete timely and accurate documentation in accordance with regulatory and payer requirements. Educate patients, families, caregivers, and facility staff on treatment strategies and recommendations. Collaborate with physicians, nursing staff, rehabilitation professionals, and other healthcare providers. Participate in care planning meetings and interdisciplinary team conferences. Ensure compliance with Medicare, Medicaid, state, and company policies and procedures. Travel between assigned facilities, patient homes, and outpatient locations as needed. Qualifications Master's Degree in Speech-Language Pathology from an accredited program. Current state Speech-Language Pathologist license. Certificate of Clinical Competence (CCC-SLP) preferred Current CPR certification or ability to obtain upon hire. Valid driver's license, reliable transportation, and automobile insurance. Strong clinical, communication, and organizational skills. Experience in geriatrics, skilled nursing, home health, or outpatient rehabilitation preferred.
