What You'll Do:
Provide therapy in patients' homes for both pediatric and now expanding into adult populations Develop individualized care plans and document progress according to state and federal regulations Collaborate with nurses, case managers, and families to optimize patient outcomes Participate in team meetings and contribute to the agency's clinical excellence Maintain compliance with Medicaid, Medicare, and private pay documentation and billing standards
Qualifications:
Licensed Occupational Therapist (OT) , Speech Therapist (ST) in Colorado Experience in pediatric or adult home health preferred Strong interpersonal skills and ability to educate families and patients Reliable transportation and willingness to travel for home visits Passion for improving patients' quality of life
