Speech Language Pathologist (PART TIME) Oak Park Nursing and Rehabilitation - 2.1 San Antonio, TX Job Details Part-time 12 hours ago Qualifications CPR Certification Certificate of Clinical Competence in Speech-Language Pathology Speech-Language Pathology License Speech therapy Full Job Description The Speech-Language Pathologist (SLP) is responsible for evaluating, diagnosing, and treating residents with speech, language, voice, swallowing, and cognitive-communication disorders. The SLP develops individualized treatment plans, provides therapy services, and collaborates with the interdisciplinary team to help residents achieve their highest level of function and quality of life.
  • Responsibilities:
  • Evaluate residents for speech, language, cognitive, and swallowing deficits.
  • Develop and implement individualized treatment plans.
  • Provide skilled therapy services in accordance with physician orders and facility policies.
  • Complete timely and accurate documentation, evaluations, progress notes, and discharge summaries.
  • Collaborate with nursing, therapy staff, physicians, residents, and families regarding treatment goals and progress.
  • Participate in care planning and interdisciplinary team meetings.
  • Ensure compliance with federal, state, and facility regulations.
  • Qualifications:
  • Current Texas Speech-Language Pathologist license.
  • Experience in skilled nursing, rehabilitation, or long-term care preferred.
  • Strong communication, organizational, and documentation skills.
License/Certification:
Speech Language Pathology License (Required)
