Services are provided in home health settings and focus on helping patients achieve greater independence, safety, and quality of life. Responsibilities Evaluate speech, language, communication, cognitive, voice, and swallowing disorders Develop individualized treatment plans based on physician orders and patient needs Provide evidence-based therapeutic interventions to improve communication and swallowing function Assess and treat patients experiencing speech, language, cognitive, and dysphagia-related challenges resulting from illness, injury, or neurological conditions Educate patients, family members, and caregivers on treatment strategies and home exercise programs Recommend communication aids, adaptive devices, and compensatory strategies as appropriate Monitor patient progress and modify treatment plans as needed to achieve therapeutic goals Collaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate care Communicate changes in patient condition and treatment response to the physician and Director of Nursing Complete accurate clinical documentation, progress reports, and discharge summaries in a timely manner Treat clients, families, caregivers, and team members with professionalism, courtesy, and respect Qualifications Master's Degree in Speech-Language Pathology from an accredited program Current Speech-Language Pathologist license in the state where services are provided Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred or required based on state regulations Minimum of two (2) years of experience as a Speech-Language Pathologist Home health or community-based experience preferred Experience evaluating and treating patients with communication, cognitive, and swallowing disorders Strong clinical assessment, communication, and documentation skills Commitment to maintaining HIPAA compliance and patient confidentiality Join Our Team If you're passionate about helping patients improve communication, cognitive function, and quality of life while enjoying the flexibility to create your own schedule, we'd love to hear from you.
